Humana's Commitment to Privacy Practices

Relationships are built on trust. One of the most important elements of trust is respect for an individual's privacy. We at Humana value our relationship with you, and we take your personal privacy seriously.

Humana has established one of the most effective privacy operations in the industry, working throughout the company to assure compliance with privacy provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), a federal law designed to ensure the privacy of personal and health information that took effect April 14, 2003.

Humana Notice of Privacy Practice
Members enrolled in employer group medical plans, HumanaDental plans and/or individual health plans should review the documents below for information on how Humana uses and/or discloses their personal, health and financial information and the various privacy rights available to them.

Note: The above notices do not cover Administrative Services Only (ASO) self-funded employer group sponsored health plans. Members should direct their requests for the appropriate Notice of Privacy Practices to their assigned benefits administrator or Privacy Officer.

Members enrolled in governmental health plans including Medicare, Medicaid and Federal Employees health plans should review the documents below for information on how Humana uses and/or discloses their personal and health information and the various privacy rights available to them.

Individual Privacy Rights
Privacy laws, rules, and regulations provide our members with various options, which are called individual rights. Individual rights may be invoked by the member or, if appropriate documentation is provided, authorized personal representatives. The following is a brief description of the various individual rights and the appropriate form to invoke one of these rights:
