Why can't I view my spouse's claim history?
Privacy laws prevent a person from viewing information on a dependent/spouse over the age of 18. With the member's permission, another person on the policy (age 18 or older) can view the page and even perform functions on the member's behalf. To allow access, members simply log in, click on "Change My Preferences," and then choose "Manage Family Member Access."

How do I order an ID card or order a replacement ID card?
You can order a replacement ID card by registering and logging in to your personal homepage or by calling 1-800-233-4013.

How do I change an address on the Internet?
Currently, addresses cannot be changed on the Internet. Please call 1-800-233-4013 to update your address.

How do I submit a dental claim and where is the claim form?

Can I refer a dentist who is not in your network?
Yes. You can refer a dentist online or call 1-800-233-4013.

How many cleanings do HumanaDental plans cover?
Two cleanings per calendar year.

When do I need an estimate?
Estimates are not required in most cases, but are strongly recommended when services will exceed $300. You should work with your dentist to obtain this information. If you are unsure whether an estimate is required in your case, contact us via secure online messaging or call us at 1-800-233-4013.

What are waiting periods?
The time a member must wait from the effective date of coverage for certain procedures to be covered. Waiting periods depend on the size of the group and whether the group had prior dental coverage.

What is an alternate service?
The least expense service when two or more services would correct the same dental condition.

What is an annual maximum?
The amount that may be payable for each covered person for expenses incurred. No further benefits are payable after the maximum benefit is reached.

What is coinsurance?
The percentage of covered charges HumanaDental will pay after the deductible is satisfied and until the annual maximum is met.

What is a deductible?
The portion of the covered expenses that a member pays every year before HumanaDental pays benefits.

Do I pay my deductible to the insurance company or to the dentist?
A deductible is subtracted from any money owed by HumanaDental when a claim is processed.

Where do I find a claim form? How are claims submitted?
We do not require a specific claim form. Most dentist send claims to us on behalf of the patient. If you need to submit a claim, send an itemized bill, including the diagnosis, to:

How do I get an individual plan quote for my family or me?
There are two different options for an individual dental plan.
Individual Dental coverage and individual health insurance from HumanaOne.
This individual dental coverage is only available to people who purchase an individual health insurance plan from HumanaOne. For more information or a quote, please visit www.Humana-One.com or call our enrollment area at 1-866-672-9165.
Standalone Individual Dental coverage from Humana/Compbenefits.

How do I add dental to my plan?
How do I add dental to my Humana Medicare plan?
Please call Humana Medicare Enrollment at 877-877-5104 for information about our dental plan.
How do I add dental to my HumanOne plan?
At renewal, you will receive a letter advising you if dental coverage is available. If you have any further questions or would like to know your renewal date, please call HumanaOne Billing and Enrollment at 1-800-458-1354.
How do I add dental to my Humana medical plan?
Not all group medical plans have dental coverage. Please contact your Humana Resources person to see if the coverage is available.
